I can't throw grenade

I can throw grenade at the beginning of play, when the character is respawned, I can't thrown. How to fix it? Please help me.
 
Can you reproduce it within the demo scene? From the grenades perspective there isn't anything different about after the character has respawned as long as you have enough grenade ItemTypes. This video goes into detail:
